HEMBRO: Hemorrhoid Embolization Through the Foot

Dr. Rishi R. Sood introduced Pedal HEMBRO, his novel approach to hemorrhoidal artery embolization performed through a small artery near the foot or ankle.

HEMBRO is a minimally invasive, image-guided treatment for symptomatic internal hemorrhoids. Using a tiny catheter, Dr. Sood selectively reduces excessive blood flow to the hemorrhoidal tissue—without rectal incisions, stitches, or surgical removal of tissue.

Hemorrhoidal artery embolization has traditionally been performed through an artery in the groin or wrist. With Pedal HEMBRO, Dr. Sood instead uses a distal tibial artery near the ankle. This avoids both groin access and post-procedure wrist restrictions while allowing the patient to walk immediately after treatment.

Dr. Sood performed his first Pedal HEMBRO procedure for a patient with recurrent hemorrhoidal bleeding despite three previous rubber-band ligation procedures. Multiple hemorrhoidal arterial branches were successfully treated using only 22 mL of iodinated contrast. The patient walked immediately afterward and returned home following a brief outpatient recovery.

Advantages of the Pedal HEMBRO Approach

  • No groin arterial puncture
  • No post-procedure wrist restrictions
  • No rectal incision or surgical tissue removal
  • Immediate walking after treatment
  • Brief outpatient recovery
  • Same-day discharge
  • An additional option after unsuccessful conservative treatment or banding

Pedal HEMBRO may be considered for appropriately selected patients with persistent bleeding from internal hemorrhoids. Every patient requires an individualized evaluation, and rectal bleeding must first be properly investigated to exclude other potential causes.

Pedal HEMBRO is a novel access technique. Contrast utilization, procedure time, recovery, symptom improvement, and clinical outcomes vary according to each patient’s anatomy and medical condition.
